AMBULANCE SERVICES
The Central Arizona Fire and Medical Authority (CAFMA) launched its ambulance transport service on October 1, 2024. CAFMA’s ambulances operate 24/7 with a skilled team of paramedics and emergency medical technicians (EMTs) dedicated to safeguarding the health and well-being of community members in Prescott Valley, Chino Valley, Dewey-Humboldt, and surrounding areas.
As an ambulance provider in the state of Arizona, CAFMA is regulated by the Arizona Department of Health Services Bureau of EMS, and we operate within the parameters of our Certificate of Necessity (CON).

RECORDS REQUESTS
Community members can request EMS or fire incident reports documenting calls and/or incidents responded to by CAFMA personnel. These reports provide valuable details about specific emergencies and can be accessed by completing a Records Request Form. - +How can I pay my ambulance bill?
We offer multiple ways for you to pay your ambulance bill. Please have your account number from your statement ready.

- +How much will an ambulance transport cost?
Ambulance billing rates are determined by the Arizona Department of Health Services (DHS) and can be viewed on the DHS website under Historical Rate Schedules. Select the current month to see current rates of ambulance providers across Arizona, including CAFMA.
The rate for each transport varies, depending on treatment necessary for the patient and distance traveled with the patient to the hospital.
- +Why isn't the cost of an ambulance covered by my taxes?
Ambulance service in the state of Arizona is heavily regulated. All ambulance service providers in Arizona, including ambulances run by city or town fire departments, fire districts, or private companies, have transport fees that are set and regulated by the Arizona Department of Health Services Bureau of EMS and Trauma. These fees are separate and distinct from property taxes under the law.
- +Does CAFMA offer a discount if I pay in full or don't have insurance?
Arizona Revised Statute 36-2239 section D states “…an ambulance service shall not charge, demand or collect any remuneration for any service greater or less than or different from the rate or charge determined and fixed by the department as the rate or charge for that service.”
As such, it is against the law for CAFMA to offer a discount on an ambulance bill.
- +Why didn't my insurance pay for my ambulance transport?
CAFMA is required by law to bill for providing an ambulance transport. You may receive a statement for one or more of the following reasons:
- We may need to contact you for your insurance information, auto liability, or workers’ compensation information. You may submit your insurance information via this website under Submit Insurance Info.
- We may also need to obtain a signature authorizing us to submit a claim to your insurance on your behalf. You may submit the signature authorization form via this website under Visit Patient Portal.
- There may also be a cost-sharing (coinsurance, copay, deductible, etc.) amount that you may be responsible for. If you feel that your insurance has incorrectly processed your claim, please contact your insurance company for further information.
